Second or third stage, which would detach somewhere over the central Pacific. That's not uncommon, that rockets go out of control into spiral patterns. Doesn't happen every day, but you see it every few months or so, where a stage detaches with some remaining fuel and thrust available, and ends up flying in a spiral pattern.
