Regarding the first part: rule change a few years ago.  QB is down when he starts to give himself up.

But even before that...the entire OL was stood up and collected into a bag.  Heinecke ran right in to the back of them, and had absolutely nothing there.  Milano cleaned up, but it was great line play.
